Other than that, considering your elderly parent is in a relatively healthy condition who does not require such specialized support, our basic caregiver can be very helpful in such scenarios.
Functional Decline Without Acute Illness (The “Frail Elderly”): Not every medical problem arrives with an ambulance. Many older adults experience a slow, creeping decline: they stop cooking because standing is hard, stop bathing because the tub is slippery, stop taking medications because the pill organizer is confusing. This is not an emergency—yet. But without intervention, it will lead to a fall, malnutrition, or a urinary tract infection that becomes sepsis. In-home patient care is appropriate the moment a family notices weight loss, unpaid bills, soiled clothing, or a cluttered, unsafe home. A Case Study:
Let me share an experience with a close patient of ours. Today the patient is totally bedridden and needs everything done for him by an experienced caregiver. 3 years ago he was in much better physical condition. Though he already had dementia and cognitive issues, he could still walk and do much of his personal stuff on his own. But people needed to check on him on an hourly basis and he needed constant reminder of certain things. The adult children did as much as they could but it was an impossible job for them to do it without fail all the time while maintaining their own life.
What they needed was a sincere basic caregiver, whose main job would have been to keep watchful eyes, his presence would have solved much of their problem. The caregiver did not necessarily have to be an expert in health-care, but he needed to be sincere helper who would faithfully follow your instructions. It didn’t necessarily had to be 24 hours full-time job, even a 12 hours daily shift would have helped the family out, 4/5 days a week would have been better than no help.
The family originally didn’t know about care-giving services/agencies. Choosing the right caregiver for elderly care.
Mainly 3 types of caregivers provide elderly care,
- diploma nurse,
- patient care attendant(PCA),
- Basic caregiver.
They all have their advantages and disadvantages. Nurses are most well trained to handle all kind of healthcare related scenarios, from simple to complex, which makes their services relatively costly. PCA are also well trained but not on the level of diploma nurses, which is also reflected on their salary. Then we have our basic caregivers who are least trained and their service is also most affordable. These basic caregivers have in-house care-giving training, they can measure blood-pressure, blood glucose, etc.
As of 2026, if you have 25000 taka monthly budget then you can probably hire a diploma nurse on a monthly basis for 12 hour shift daily, you can hire a PCA for the same duration for little bit less, but you can hire a FULL time basic caregiver on that budget.. General rule of thumb:
- if you need Specialized care or if you have extra budget, consider diploma nurse,
- if you have budget constraint or if the client is relatively healthy, consider basic caregiver.
- For anything in-between, PCA can be considered.
But there are nuances to this, PCA and basic caregivers can not provide quite a few services that a diploma nurse can.
