Why did we start this caregiver agency?
I’m Mesbah, an IT professional. A few years ago, I was looking for a caregiver for my father. In the early stages, dealing with various caregiver agencies for almost a year led to many bitter experiences. Eventually, I met Saidur—a rural doctor. He started taking care of my father. One day, he shared his own bitter experiences with caregiver agencies.
That’s when we realized: despite having dozens of caregiver agencies in Dhaka, this sector still has immense room for improvement. We decided to build an agency that prioritizes the interests of both clients and caregivers. This would be an organization formed by care recipients and caregivers—for care recipients and caregivers.
The challenges We Faced with Traditional Agencies
To make the picture clear, let me share the problems Mesbah (as a client) and Saidur (as a caregiver) personally faced.
First: When my father suddenly fell ill, we needed a caregiver urgently. Initially, we didn’t know about agencies—we directly contacted clinic PCAs and hired a caregiver at a very high cost. Later, we hired from an agency. Though the quality was acceptable, we were completely unaware of market rates, so we overpaid for the first two months.
Second: When we approached another agency, new problems appeared. Their charges were relatively lower, but they sent us a completely inexperienced young man. We were paying the agency 27,000 BDT, out of which they gave only 16,000 BDT to the caregiver—pocketing 11,000 BDT themselves. In other words, we were paying for a skilled caregiver but receiving a novice, while the agency kept the profit.
Frustrated, we hired someone privately outside any agency. He stayed for a few months, but suddenly left when he found a “better” job—leaving us stranded once again. Hiring via agency gave one set of problems; hiring privately gave another. This is when I met Saidur.
Third: After Saidur took over my father’s care, I learned that he had helped many people get placements through agencies, and in most cases, the agencies kept a considerable portion of the salary. So, on one hand, patients do not receive quality care proportionate to what they pay. On the other hand, skilled caregivers are not paid according to their qualifications. When a major share of a caregiver’s earnings goes to the agency, frustration naturally sets in—affecting their work quality. Moreover, worrying about how to run a household on low pay makes it impossible to serve wholeheartedly. In short, under the current business model, neither the service recipient nor the service provider benefits—only the agency does.
Forth: Many agencies lack sufficient manpower. So they assign the same caregiver to tasks of varying importance. Sometimes they send a novice for complex cases; other times, an overqualified person for simple tasks—always charging the same high fee. They send whoever is free at the moment, ignoring the patient’s best interest.
For example, an elderly acquaintance of mine needed a caregiver. He was generally healthy but suffered from age-related weakness. An sincere, novice caregiver would have been sufficient for his daily needs. Yet the agency demanded the fee of a highly skilled caregiver—beyond the family’s budget. As a result, they were deprived of timely service.
In many cases, agencies do not maintain a separate category of low-trained, lower-wage caregivers specifically for healthy elderly individuals. Moreover, proper placement of diploma nurses and PCAs based on patient needs is often missing. Instead, agencies prioritize their own internal or mandatory people—not based on patient needs or caregiver qualifications, but because profit margins are higher.
In short, the inability or unwillingness to provide suitable caregivers based on a patient’s budget and medical importance is a major failure and limitation of most existing agencies today.

The main cornerstone of our difference lies in our economic model. We have observed a cycle in the local caregiving industry where high commission fees lead to caregiver burnout and turnover. At BDHomecare, we take a deliberately smaller cut from our service fees. This means two vital things for families across Bangladesh: first, our services remain more affordable without compromising on quality or safety standards. Second, and perhaps more importantly, our caregivers take home the lion’s share of the bill. A caregiver who feels valued, fairly compensated, and not cheated by the system arrives at your home in Dhanmondi or Uttara with a genuine smile and a commitment to excellence. We firmly believe that ethical business practices—treating both the client and the caregiver with morality and respect—are not just good karma; they are the foundation for long-term, sustainable service. By earning a smaller profit on more families served, we ensure that the standard of care in Bangladesh rises for everyone.
Tailored Care, Not a One-Size-Fits-All Approach
Finding reliable home care is not just about filling a shift—it is about finding the right fit for your loved one. At BDHomecare, we operate on a philosophy that sets us apart from the conventional agency model in Bangladesh: we prioritize precision matching over generic placement. We understand that the needs of a senior parent requiring companionship and gentle oversight in Gulshan / Banani differ vastly from the needs of a patient in Mirpur / Mohammadpur recovering from surgery who requires a trained Diploma Nurse. As we explained earlier, many caregiver agencies, constrained by a limited roster of available staff, resort to sending whoever is free, often resulting in a mismatch—either an overqualified professional charging a premium for basic elderly care, or an under-prepared aide tasked with complex medical needs. At BDHomecare, we are actively building the largest and most diverse digital caregiver database in Bangladesh. This infrastructure allows us to perform a level of match-making that ensures you receive the precise level of care required—whether it’s a compassionate Personal Care Assistant (PCA) or a highly skilled Diploma Nurse—at the price point that accurately reflects the service. We also plan to always maintain a separate category of less-trained, lower-wage caregivers specifically for relatively healthy elderly individuals as described in our services list.

Measures taken to Ensure Client Safety and Quality of Care

At BD Homecare, we ensure the safety and quality of our services for clients through a rigorous three-stage screening and final merit-ranking process for all caregivers. Each of these stages holds distinct importance.
1) Security Screening
Ensuring client safety is our foremost commitment. All caregivers are required to submit copies of their National ID (NID), photographs, and full contact addresses. We verify the authenticity of these documents, and only upon completion of this step can a caregiver become a member of BD Homecare. However, beyond this, we proactively encourage caregivers to complete additional voluntary steps and provide further documentation. Those who fulfill these additional criteria receive a higher security clearance and are prioritized for assignments. These optional steps are outlined below:
- Police Clearance Certificate: Caregivers who submit this certificate are placed higher on our security list. For sensitive cases, only caregivers with a submitted police clearance are assigned. While currently voluntary, the submission of a police clearance certificate will soon become mandatory.
- Testimonials from Two Acquaintances: The caregiver provides the names, addresses, and phone numbers of two contacts. We optionally speak with these individuals to assess the caregiver’s social standing. We believe that individuals respected within their community and family are less likely to engage in misconduct, as the fear of social and familial disgrace plays a significant deterrent role.
Caregivers who complete the mandatory first step along with these two subsequent voluntary steps receive our highest security clearance. Furthermore, for our “Couple Caregiver” package, the marriage certificates of caregivers identifying as a husband-wife pair are mandatorily verified.
2) Verification of Training, Experience, and Skills
All caregivers submit copies of their training certificates and documentation of work experience in the care sector. After reviewing these, we create a preliminary merit ranking. For those without formal training or experience, we prepare them as Entry-Level Basic Caregivers through our in-house training program, retaining only the competent individuals for potential assignments.
3) Client Feedback on Caregivers
Following the first two stages of security clearance and training verification, feedback from care recipients plays a crucial role in determining the true quality of a caregiver. We maintain regular contact with the client’s family during the service period. During or after the service term, we inquire about the quality of care, asking about the caregiver’s honesty, sincerity, proactivity, attitude, willingness to follow instructions and performance.
This feedback, both positive and negative, is added to the caregiver’s profile and contributes to the final merit ranking. Any areas for improvement identified at this stage are noted, and we assist the caregiver in addressing these weaknesses. However, a caregiver who consistently demonstrates shortcomings is ultimately dismissed. This ensures that we retain only acceptable, high-quality “apples” in our basket, with no room for substandard ones.
4) Final Merit List
Finally at the fourth stage, we take all the results and feedback from the first three stages and produce the final caregiver ranking. Which is also an ongoing process as database is updated after each contract ends. Through this step-by-step screening process, we are actively building an extensive database of caregivers and their merits, which helps guaranteeing the standard of care provided.
